BEFORE ME, the undersigned authority, personally appeared Richard D. Murphy, who being duly sworn says:
  

He is an agent of CHAPMAN GROVES HOMEOWNERS ASSOCIATION, INC, the lienor herein, hereinafter "Association", whose address is 1633 E. Vine Street, Suite 110, Kissimmee, FL 34744.

That, XXXXXXXXX XXXX, hereinafter "Owners", are the record owners of the following described real property in Seminole County, Florida:

Lot XX, CHAPMAN GROVES, according to the Plat thereof as recorded in Plat Book 48, Pages 44-46, of the Public Records of Seminole County, Florida.

That owners are delinquent in the payment of homeowner's association assessments in the total sum of $140.00 as of the date hereof, together with interest from the dates due at the rate of eighteen percent (18%).

The notification of said delinquency has been given, but the same remains unpaid as of the date hereof.

That the declaration of Covenants, Conditions and Restrictions, recorded at O.R. Book 2857, Page 0370, Seminole County, Florida Public Records, provides that a lien for unpaid assessments may be placed upom lots in the Chapman Groves subdivision.

WHEREFORE, notice is hereby given that Association claims a lien in the amount of $140.00 on the real property of Owners described above, which lien shall also secure all late fees, interest, cost of collection, including reasonable attorney's fees, and all assessments to become due and payable henceforth until payment in full of same.

That, XXXXXXXXXXXXX, hereinafter "Owners", are the record owners of the following described real property in Seminole County, Florida:

UNIT XXXX, NORTHLAKE VILLAGE BUILDING CONDO 19 1906, according to the public plat thereof, as recorded in Plat Book 41 pages 74-77 of public records of Seminole County, Fl.

That owners are delinquent in the payment of homeowner's association assessments in the total sum of $2,922.00 as of the date hereof, together with interest from the dates due at the rate of six percent per annum.

The notification of said delinquency has been given, but the same remains unpaid as of the date hereof.

Section 718.116 of the Florida Statutes as well as the Declaration of Condominium of NORTHLAKE VILLAGE COMMUNITY ASSOCIATION recorded in OR Book 1621, Pages 0335-0356 Seminole County, FL, provides that a lien  for unpaid assessments may be placed on units at NORTHLAKE VILLAGE CONDOMINUMS.
